CASE MGMT RN needed in Manitowoc, WI - The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating medical services for a patient. It is important that RN Case Managers focus on providing quality medical services and efficient processes for the patient and organization.
Responsibilities
A RN Case Manager's duties include:

Talking with the patient and their family members to diagnose their needs.
Creating a plan to satisfy the patients needs.
Providing services to the patient and their family.
Assisting in locating the most cost-effective method of care.
Ensuring the plan is exceeding the needs of the patient.

Requirements
One year of experience as a Registered Nurse Case Manager within the past two years.

Active RN License in good standing.
Up to date Basic Life Support through American Heart Association.
Must pass pre-employment drug screen.

Requirements
