CASE MGMT RN needed in Lebanon, NH - The RN Case Manager is responsible for planning, coordinating and facilitating necessary services for patients. It is crucial that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the patient and organization.
Responsibilities
A RN Case Manager's responsibilities consist of:

Meeting with the patient and their family to determine their needs.
Establishing strategies to satisfy the patients needs.
Providing assistance to the patient and their family members.
Help in locating the lowest cost method of care.
Reaching out to ensure the plan is meeting the patients needs.

Requirements
One year of experience as a RN Case Manager within the past two years.

Active Nursing License in good standing.
Up to date BLS through American Heart Association.
Must pass pre-employment drug screen.

New Hampshire was one of the original thirteen colonies that formed the United States and it's rich in history. One example is just outside of Portsmouth is the Wentworth-Coolidge Mansion, the former home of New Hampshire's first royal governor, Benning Wentworth, who served from 1741 to 1767. The mansion overlooks Little Harbor and is one of the most well preserved homes of the colonial era.


The White Mountains cover about a quarter of New Hampshire and they contain the tallest peaks east of the Rocky Mountains. The highest is Mount Washington at 6,288 feet and it winds over 230 mph above timberline. In the summer the White Mountains are great for hiking and in the winter you'll find great skiing, snowboard and tubing at the many ski resorts. But in the fall, the countryside is redecorated in a myriad of vibrant colors and people come from across the country to hike and drive among the fall foliage.



The land is beautiful, the people are independent and friendly and the pace is comfortable. New Hampshire is a good place to advance your nursing career.

Requirements
