CASE MGMT RN needed in Bakersfield, CA - The RN Case Manager is responsible for planning, coordinating and facilitating service for a patient. It is important that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the organization and patient.
Responsibilities
A RN Case Manager's duties include:

Talking with the patient and their family to determine their needs.
Establishing strategies to satisfy the patients needs.
Providing resources to the patient and their family members.
Assisting in locating the most affordable method of care.
Reaching out to ensure the plan is exceeding the needs of the patient.

Requirements
One year of experience as a Registered Nurse Case Manager within the past two years.

Active Nursing License in good standing.
Current Basic Life Support from American Heart Association.
Negative Drug Screen.
