Case Management RN needed in Escondido, CA - The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating medical services for a patient. It is important that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the organization and patient.
Responsibilities
A RN Case Manager's responsibilities consist of:

Meeting with the patient and their family members to identify their needs.
Establishing strategies to satisfy the needs of the patient.
Providing services to the patient and their family.
Assisting in finding the most cost-effective method of care.
Reaching out to ensure the plan is exceeding the patients needs.

Requirements
One year of experience as a Registered Nurse Case Manager within the past two years.

Active RN License in good standing.
Up to date BLS through American Heart Association.
Negative Drug Screen.

Requirements
