Position Summary:

Cedar Hills Hospital - located just 15 minutes from downtown Portland - is an 89-bed, private psychiatric hospital specializing in mental health and chemical dependency care. We provide a wide range of services and programs that offer evidence-based treatment proven to have positive outcomes for our patients. Our inpatient and outpatient services offer supportive and compassionate care through specialty programs that are tailored to meet the needs of our patients.

The Case Manager is responsible for the following:

-Managing the treatment activities offered to the patient, which may include the appropriate clinical screening of potential admissions, establishment of treatment plans, demonstrates effective communication to support and interface with the treatment team, external case managers and managed care organizations. Also impacts implementation and supervision which accomplishes all aspects of the discharge planning process.
-Performing the utilization review/management function in accordance with the facility’s Utilization Review and Management Plan. This includes, but is not limited to, completing admission and continued stay reviews within the required timelines and utilizing established admission and continued stay criterion as well as communicating findings of these reviews to the attending physician and other members of the treatment team as appropriate.
-Maintains current knowledge of facility’s Utilization Review and Management Plan and established admission and continued stay medical necessity criterion which may be specific to various individual payers with whom the facility contracts.

Education: Master’s Degree in Social Work, Psychology, Counseling or related Mental Health Field, or Registered Nurse.
Experience: 2-5 experience in clinical Mental Health setting. Case Management experience preferred.
Licensure: Must be eligible for licensure in stated field.
Training: Successful completion of CPR Certification. Company-provided training including Crisis Management and Service Excellence must occur within 90 days of employment.
