Position Summary:

Under the supervision of the Chief Executive Officer and in collaboration with RN Case Management Director, Discharge Planner, and Social Services provides case management services including concurrent review of medical records, admission status and/or length of stay, over/under utilization and resource management or quality issues for inpatients and outpatients receiving care at Desert View Hospital (DVH). This position also provides educational support and other duties under the direction of the Chief Nursing Officer and the Chief Executive Officer.

Serves as a patient advocate.

As advocate, works to secure funding and/or coordinate resources for under-funded patients as necessary through financial counseling services.

Collaborates and communicates with patients, families, physicians, all interdisciplinary team members and outpatient treatment team, to encourage the highest level of patient independence possible.

Using evidenced-based criteria, screens patients for appropriate admission and level of care status.


Certification, Registration, or Licensure: Current Nevada R.N. license, BLS certification required. Accredited Case Management certification encouraged within 2 years of hire.

Minimum Education: Graduate of accredited School of Nursing. BSN preferred.           ; ; ; ; ; ;      

Minimum Experience: Minimum of three years case management experience in the acute hospital setting preferred but not required. Substantial clinical nursing experience with considerable background in medical and nursing care practices and procedures preferred.

Other: Clinical experience requirements are dependent upon specific position. Required to be flexible in scheduling, i.e. weekends and holidays. Demonstrates organizational skills and leadership ability. Demonstrates the ability to understand the unique infant child, adolescent, adult and geriatric population and meets the needs relative to growth, assessment and development of the patients.
