Working at Oak Plains Academy
Oak Plains Academy is a comprehensive and highly structured 90-bed residential treatment center located on 25 acres of rolling hills in Ashland City, Tennessee. Oak Plains provides psychiatric and neurological treatment services for males and females, ages 5-17, who have significant history of emotional or behavioral issues. The clinical programs at Oak Plains Academy have been designed to address the needs of children and adolescents with a primary psychiatric/ neurological diagnosis. Residents receive an individualized treatment plan, geared to meet their personal needs. A professional tem of psychiatrists, psychologists, social workers, therapists, nurses, and teachers work closely with each resident to assist them to achieve there therapeutic and education goals.

Provides efficient and effective case management and coordination of services for children in the foster home program.
The foster care Case Manager II must have graduated from an accredited college or university with a Masters in Psychology, Counseling, Social Work, or related Human Services Field. One year experience in working with children is required.

Valid TN State Driver License with appropriate Endorsements.

Ability to read and interpret reports and other legal documents such as permanency plans, and adjudication orders. Ability to write reports and ability to effectively present information and respond to questions from administration, DCS, children and their families, and general public.

Physical Demands: Must be able to assist in home studies, must be able to drive up to and including 8
hours, must be able to stand for extended periods of time up to and including 8 hours, lift 75 lb. assisted or unassisted, must be able to stoop, bend, reach objects overhead, attend and manage meetings.
Visits each assigned child per policy or as needed.
During visits to assigned home, completes the "Contact Log" sheet as per documentation policy.
Coordinates treatment-planning sessions, notifies all necessary parties, manages the meeting, and documents the progress and goals discussed as per documentation policy.
Attends court hearings as appropriate.
Advocates for assigned children, acts as a resource for assigned foster parents, and provides training and consultation to foster parents when appropriate.
Ensures implementation of treatment plan; consults with Program Director regarding issues with resources necessary for plan implementation.
Develops a Family Service Plan, monitors implementation of the plan, and locates all services and placements a child and/or family may need while enrolled in the continuum.
Documents progress and problems, maintains contacts with the custody department and Resource Managers, revises the Family Service Plan as needed, maintains ongoing contacts with the child and/or family, plans and implements them.
Provides or coordinates transportation services to the child and family to ensure participation in provided services, court hearing, foster care review hearings, case related meetings, family visits and related services.
Participates in on-call schedule to provide on-call services to foster parents. Reports and documents any on-call activities the next business day. Calls Continuum Director after hours for consultation as needed.
Reports any non-compliance with regulatory agencies or policies and/or any suspicions of abuse or neglect to Continuum Director or to proper authorities.
Consistently meets facility and regulatory training requirements by attending all mandatory meetings and training sessions as required.
Use active listening skills and appropriate tone and volume of voice, while offering a variety of positive coping skills. Always provide positive reinforcements and utilize tools from the TF-CBT training to help the client regain control of their behaviors.
Other duties as assigned.
